4. UG/PG degree/provisional degree certificate and year-wise semester-wise consolidated mark sheet.
5. Transfer certificate/migration certificate or college leaving certificate
6. Nativity certificate for North or Eastern state candidates
7. Aadhaar card
8. Physical fitness certificate (as per the format)
9. Affidavit by parent and student (as per the format)
10. Undertaking in the enclosed format
11. Six recent colour-size passport photographs
